Short answer
Subjective BPPV means you have the typical brief spinning with position changes, but no nystagmus is seen during positional testing. It may reflect a small amount of debris that causes symptoms without enough eye movement to see. Studies suggest repositioning maneuvers still help many of these patients, so treatment is often worthwhile, with other causes checked if symptoms do not improve1.
Answered by Dr. Manan Patel, PT, DPT, TPS, Certified Vestibular Specialist and Therapeutic Pain Specialist. Updated October 18, 2026.
Why might BPPV not show nystagmus?
The amount of debris may be too small, the crystals may have shifted before testing, or nystagmus may be suppressed when testing is done without goggles in a bright room. Infrared video goggles reduce the chance of missing it, and repeating the test on another day sometimes reveals it2.
Is it still worth doing a repositioning maneuver?
Often, yes. When the history strongly fits BPPV, your symptoms suggest the side and no warning signs are present, a maneuver for the most likely canal is low risk and frequently helps1. We then track your symptoms over the following days.
What else can cause positional dizziness without nystagmus?
Neck-related dizziness, vestibular migraine, low blood pressure when changing position, medication effects and residual dizziness after earlier BPPV can all mimic it. If symptoms continue, we look at these with BioSway balance testing and a neck exam, and involve your physician when needed.
