How this self-check works
Choose dizziness and balance, or neck, back and spine pain. The check starts with safety questions that look for warning signs, then asks how your symptoms feel, when they happen and what goes with them. It ends with the conditions your answers resemble, who else should be involved, and what a first visit looks at.
It runs entirely in your browser. Your answers are not saved or sent anywhere, and you can copy or print a summary to bring to your appointment. We only receive what you type into the appointment form.
It is a starting point, not a diagnosis. Several conditions overlap, and only an in-person evaluation can tell what is causing your symptoms.
