Sign and bring it

What to bring to your first visit
- Your completed new patient packet
- A photo ID and your insurance card
- A list of your medications, or the bottles
- Any imaging or test reports, such as MRI or hearing test results
- For a car accident or work injury: your claim number and your attorney or adjuster's contact details
- Comfortable clothes and flat, supportive shoes. If your dizziness is strong, arrange a ride.
