In short
Cervicogenic dizziness is unsteadiness linked to neck pain or a neck injury such as whiplash. Faulty signals from the upper neck can disturb your sense of balance. It is diagnosed by ruling out other causes and treated with neck and balance therapy.

Common symptoms
- Unsteadiness or light-headedness that comes with neck pain or stiffness
- Symptoms that build with sustained neck positions
- Headache starting at the base of the skull
- Often a history of whiplash or neck injury

What causes cervicogenic dizziness?
Whiplash and other neck injuries, neck arthritis or long periods in a strained posture can disturb the position signals the upper neck sends to the brain's balance centers.
Neck-related dizziness often occurs alongside a concussion after whiplash. Because the symptoms can look similar, we also check for BPPV before treating the neck.
Neck pain or headaches, too?
Neck-related dizziness usually comes with neck pain, stiffness or headaches. As a Therapeutic Pain Specialist, Dr. Patel treats the neck as part of the same plan.
How we test
Your first visit is a full 60-minute, 1-on-1 evaluation, and treatment often starts the same day. Depending on your symptoms, testing can include:
- Screening for inner-ear and neurological causes first
- Neck movement, strength and joint position sense testing
- Smooth pursuit neck torsion testing
- Balance testing
What is Cervicogenic Dizziness?
Cervicogenic Dizziness originates from dysfunctional joint and muscle receptors in the upper cervical spine (C1–C3) rather than the inner ear. Because the upper neck contains the highest density of muscle spindles in the body, whiplash injuries, concussions, cervical arthritis, or chronic postural strain can send corrupted proprioceptive signals to the brainstem—creating a sensory mismatch with your eyes and inner ear.
How We Differentiate Neck Dizziness from Inner-Ear Vertigo
Using Infrared Video Goggles (VNG), we first rule out inner-ear crystal or nerve pathology, then perform cervical joint-position error testing and head-neck differentiation tests to isolate the exact upper cervical segments driving your unsteadiness.
Integrated Cervical & Vestibular Rehabilitation
- Targeted Manual Cervical MobilizationGentle, specific joint mobilization and myofascial release to normalize upper cervical facet mechanics and calm hyperactive suboccipital mechanoreceptors.
- Cervical Joint Position Error (JPE) RetrainingPrecision head-repositioning drills that recalibrate your brain's internal map of where your head is oriented in space.
- Deep Cervical Flexor & Oculomotor IntegrationSynchronizing deep neck stabilizer endurance with Cervico-Ocular Reflex (COR) and smooth-pursuit eye tracking.

Your treatment plan
Rule out other causes
Because cervicogenic dizziness is a diagnosis of exclusion, we screen carefully for inner-ear and other causes before treating the neck.
Treat the neck
Manual therapy and targeted exercise to restore neck motion and reduce pain.
Retrain position sense and balance
Exercises that sharpen the neck's position sense and connect it with eye and balance control.
Recovery and what to expect
When the neck is the main driver, dizziness often improves as neck pain and movement improve. Careful screening first makes sure nothing else is missed.
Tips for daily life
- Break up long stretches of screen work
- Set your monitor at eye level
- Keep gentle neck movement in your day
- Tell your doctor about any numbness, arm weakness or severe headache

Questions about cervicogenic dizziness
Is this the same as vertigo?
Cervicogenic dizziness is usually a sense of unsteadiness rather than true spinning.
Can a car accident cause dizziness?
Yes. Whiplash is a common trigger. We accept auto-accident attorney liens and Workers' Compensation.
